Case Coordinator

Aspire Living & Learning is dedicated to supporting neurodiverse individuals across New England and Maryland. The Case Coordinator will manage cases under supervision, collaborating with team members to provide necessary support and ensure effective program implementation. Responsibilities The Case Coordinator carries a caseload assigned by the supervisor, director, or department head. Individuals on the caseload may have varying degrees of need or oversight. Below outlines key aspects of the roles and responsibilities this position holds: Demonstrates the skills to work with a variety of individuals of varying ages and functioning level Dependent on service line, develops, oversees implementation of behavior, treatment, educational, or medication support plans, as applicable Assists in the modeling and teaching of behavioral or mental health strategies to necessary stakeholders (e.g., staff, providers) Trains the managers, DSP leads, etc. on any plans, or individual specific training, when applicable Monitors, verifies, and documents various levels of programmatic or clinical support or service provision. This may include collecting, aggregating, or reporting out on service specific data points or data collection procedures. Ensures accurate and timely reporting Completes all required documentation Meets deadlines Ensures relevant sections of electronic record is up to date and accurate Facilitates or attends all team meetings Effectively communicates in both verbal and written form with necessary stakeholders Accepts feedback from supervisors to ensure the quality of program implementation Communicates and collaborates with both internal and external team members and stakeholders Maintains ethical and professional boundaries and guidelines Works cooperatively and collaboratively with all necessary stakeholders Cultivate a positive and productive workplace atmosphere to enhance team morale and productivity Participate in and complete training sessions as delegated by management to enhance skills and knowledge relevant to the role Conduct training as required and assigned Perform any additional job responsibilities assigned by supervisors or team leaders to support the organization's smooth operation Accept reassignment as required Skills High school diploma or GED required, bachelor's preferred Must meet the Registered Behavior Technician requirements Excellent interpersonal, organizational, and communication skills Computer skills Must have satisfactory background checks Valid driver's license and safe driving record Benefits Comprehensive Health, Dental, & Vision Insurance - Choose from multiple medical plans, with vision coverage included at no extra cost.
