Communicate effectively with members, their families, and healthcare providers to ensure effective care coordination. Collaborate with other members of the healthcare team to develop and implement comprehensive care plans. Monitor and evaluate the effectiveness of care plans and make necessary adjustments as needed. Conduct thorough assessments of members' medical conditions and identify potential barriers to care. Develop and maintain positive relationships with members to promote trust and engagement in their care. Ensure that all care provided meets regulatory standards and follows evidence-based practices. Document all care and interventions accurately and in a timely manner. Stay up-to-date on industry trends, best practices, and new developments in healthcare to continuously improve care delivery. Adhere to Aetna's policies, procedures, and standards of practice. Maintain confidentiality and privacy of member information. Advocate for members' needs and rights. Participate in team meetings and contribute to the development of Aetna's policies and procedures. Serve as a positive representative of Aetna to members and the community. Maintain a positive and professional attitude while providing care. Continuously reflect on and evaluate own performance to identify areas for improvement. Adhere to all safety protocols and guidelines. Attend trainings and professional development opportunities as required. Other duties as assigned by the supervisor.
