Implementation RN - LTSS Programs

Location: Remote with flexibility Duration: 3 to 6 month contract with potential extension Description: We are seeking a Registered Nurse (RN) with experience in Long-Term Services & Supports (LTSS) to support the successful implementation and operational rollout of care management programs for one of our clients. This role will be instrumental in translating program requirements into actionable workflows, supporting configuration and testing of the care management platform, and ensuring readiness of clinical users prior to go-live. The ideal candidate brings both clinical expertise and operational insight into Medicaid LTSS, Managed Long-Term Services and Supports (MLTSS), or Duals/D-SNP programs, with the ability to collaborate across business and technical teams. What You Will Do: • Serve as the clinical subject matter expert (SME) during implementation of the care management platform for LTSS programs. • Participate in workflow design sessions, providing input on nursing processes, assessments, and care planning within the LTSS model. • Support system configuration and UAT (User Acceptance Testing) to ensure that clinical workflows align with program and regulatory requirements. • Review and validate LTSS assessment tools, care plan templates, and task management logic in the platform. • Partner with program leadership to document and refine policies, procedures, and process maps for care coordination and service authorization. • Conduct or assist with training and onboarding of care management and utilization management nurses pre-go-live. • Provide feedback on system usability and help identify opportunities for workflow optimization. • Serve as a liaison between the vendor’s implementation team and the client’s clinical operations staff to ensure alignment between clinical practice and system design. • Support post-go-live stabilization efforts, including triaging clinical workflow issues and validating data integrity of member records. What You Will Bring: • Active, unrestricted Registered Nurse (RN) license (compact or Pennsylvania license preferred). • 3–5 years of experience in LTSS, MLTSS, or related care management programs within a Medicaid or Medicare Advantage health plan. • Prior experience in system implementation or platform transition projects (e.g., Jiva, Virtual Health, Essette, GuidingCare, MHK, TruCare, etc.) strongly preferred. • Understanding of care coordination models, service authorization processes, and state/federal LTSS requirements. • Strong analytical and communication skills, with the ability to translate clinical needs into system requirements. • Proficiency in documentation systems, care management software, and Microsoft Office Suite. • Comfortable working in a fast-paced, collaborative environment with both clinical and IT stakeholders. • Experience supporting Pennsylvania LTSS or CHC (Community HealthChoices) programs preferred. • Familiarity with EHR integration, data migration, or UAT testing protocols preferred. • Prior participation in managed care platform go-lives or training initiatives preferred.
