About the position The Patient Liaison Program Coordinator at Cleveland Clinic is responsible for enhancing patient experiences and outcomes by developing programs that focus on continuous service improvement, optimizing length of stay, and reducing re-admissions. This role involves acting as a liaison between patients and care providers, ensuring effective communication and coordination of care, and implementing processes that support patient needs. Responsibilities • Assist with program development focused on continuous service improvement. • Promote an integrated approach to manage the continuum of care for patients and their families. • Serve as a liaison to patients with internal and external care providers. • Provide input to develop standard operating procedures for patient care. • Implement new processes to guide patients to appropriate resources and support services. • Proactively round to meet new patients and promote available programs and services. • Provide education to patients and families. • Capture patterns of successes and challenges in patient experience for analysis. • Work with other areas to implement services that meet patient needs. • Identify, evaluate, and resolve patient complaints. • Document all contacts and interactions with patients. • Remove barriers to care and facilitate coordination of care. • Facilitate communication in response to patient needs. • Ensure positive working relationships between physicians, caregivers, patients, and families. • Support the highest quality of patient standards. Requirements • Associate's degree in communication, psychology, sociology, or a related field. • Three years of related experience in customer relations with diverse patient/culture populations. • High school diploma/GED and five years of progressively responsible experience may substitute the education requirement. Nice-to-haves • Bachelor's degree. • Experience working in a skilled nursing facility. • Familiarity with health system navigation and the patient experience. • Residence in the Cleveland area.
