Key Responsibilities: • Expert Triage: Conduct comprehensive telephone and video-based patient assessments using clinical protocols and critical thinking skills • Care Transitions: Manage post-discharge follow-up, medication reconciliation, and care coordination to prevent readmissions • Clinical Communication: Respond to EMR alerts, route messages appropriately, and facilitate communication between patients and providers • Documentation Excellence: Maintain detailed, accurate documentation of all patient interactions and clinical decisions • Team Collaboration: Work seamlessly with on-site clinical teams, physicians, and virtual care colleagues • Quality Assurance: Participate in quality improvement initiatives and maintain compliance with telehealth standards Requirements: Required: • Active RN license in Texas or Compact State licensure • Minimum 3-5 years of clinical experience in acute care, emergency medicine, or telehealth • Advanced proficiency with EMR systems and remote communication technologies • Exceptional critical thinking, assessment, and time management skills • Reliable high-speed internet and dedicated home office space Preferred: • Telehealth or telephone triage experience • Additional certifications in specialty areas • Bilingual capabilities (English/Spanish) • Experience with care coordination or case management
